How do I get the browser content including the window menu bar, border / border, and content below the fold?
I remember in the early days of the internet there were books that included the entire title bar of the browser window, the menu bar, and even the browser border and the content within it - not only the current content in the window, but all content even when scrolling down).
I would like to do this too ... is there an easy way to write a Win32 or Mac application to do this, or perhaps as a Firefox extension (add-on)?
(browser menu bar and bookmark bar can be real or fake - no need to show my personal bookmarks in the bookmark bar)

You can write such an application on the OS X side using the Webkit framework for Cocoa.
.NET on Windows has its own WebBrowser and WebBrowserBase classes that provide a programmable browser such as a widget. Check out the DrawToBitmap method of the WebBrowserBase class.
There are also several apps that already do this.
For Mac OS X, there is Paparazzi and the Little Snapper that can do what you want.
For Windows, check out SiteShoter and Webshot .

You can use the Firefox extension FireShot and then use Microsoft Paint and Alt-PrtScr to add the surrounding chrome.
