How to make a dynamic shape

I have a status field that has 3 options: 1) activated 2) rejected 3) pending. If the user selects rejected status, then he must enter a reason for rejection, otherwise this reason field should be hidden. Or when the user is rejected I have to redirect to another form so that I can send email using the rejection reason
