Approval workflow

I am trying to create a document approval workflow without sending any email. How can I know if a document is approved.

Yes, quite vague, but I'll give it a chance ... If you are using a working MOSS approval protocol or just content approval (available in WSS and MOSS), there will be an extra column in the list / library for approval. You can just make sure you have a view that includes that column to see the value. If you want to be alerted when this is changed, you can create a view that filters for completed ones and add an alert for items that match that view.
