IPhone Application Keyboard
An iPhone application usually consists of multiple UIViewControllers.
Each UIViewController manages the data and the view that displays the data.
UIViewControllers can be grouped with UINavigationController or UITabController, both of which add navigation controls to navigate between different UIViewControllers.
For example, a tab controller draws tabs at the bottom of the screen so you can switch between views. The navigation controller adds a back button so you can go back to the previous screen and you can add additional features like edit / add / delete buttons that work with your view.
Have a look at UINavigationController, UIViewController and maybe UITableView.
It will help if you explain a little how your form is meant to understand what the previous / next button is, for example. is it for navigating between multiple screens of the same form? I suspect UINavigationController is what you need.

If that's what you're asking for, the Previous, Next, and Done buttons seen above the toolbar in Safari are basically just UIToolbar
just above the keyboard. There is no built-in mechanism for performing actions for these buttons, so you have to write them yourself.
"Done" should cause the field to leave its first responder status; "Previous" and "Next" should change which field is the first responder. You can observe UIKeyboardWillShowNotification
and UIKeyboardWillHideNotification
to determine when to show and hide this toolbar and where to place it on the screen.
